Prepare before the event using visuals: show short, calm videos of a tooth falling out (muted, while you narrate slowly in simple words), a social story with pictures (“My tooth will wobble. It will fall. It does not hurt for long. I am okay.”), and role-play with a doll. On the day, stay calm, use few words, offer a preferred comfort item, and praise him for coping. The same visual-preparation method works for haircuts, nail cutting, and doctor visits.
