WHAT IS AN INDIVIDUALIZED EDUCATION PLAN
A written document outlining:
- Child’s learning goals
- Accommodations and modifications
- Support services (therapy, tutoring)

CREATING A SUPPORT PLAN
- Get formal diagnosis and assessment reports
- Meet with school principal and special educator
- Discuss child’s strengths and challenges
- Propose accommodations and modifications
- Create written document
- Sign and document in school records
- Review quarterly or semi-annually
